Is This Program Services Agreement for 1:1 Clients, Groups or Both?
The program services agreement can be used for both 1:1 programs and group programs. It has been designed to be easily customizable while still including all of the necessary provisions you need to protect yourself.
Can I Still Use This Agreement if I am Also a Licensed Healthcare Provider?
Yes, you can still use this agreement as long as you are not acting in the capacity of a licensed healthcare provider while you provide coaching services. However, it is advisable to seek further advice from an attorney to ensure you comply with the laws and regulations in your state.
What if I Change My Mind Can I Get A Refund?
Due to the immediate, direct access to the digital templates, we do not offer a refund of any kind. Additionally, if you sign up for a payment plan, we do not allow you to cancel the remaining payments for any reason. How Do I Access the Documents Once I Purchase Them?
A link to the documents will be sent to your email shortly after you purchase them. The link expires after 24 hours. If you would like to access the link after 24 hours click on the original link that was sent to you. It will tell you that you can no longer access that link and shortly after will send a new link to the contracts. There is no limit to the number of links that can be sent to you, but each link expires after 24 hours.
