Is This Program Services Agreement for 1:1 Clients, Groups or Both?

The program services agreement can be used for both 1:1 programs and group programs. It has been designed to be easily customizable while still including all of the necessary provisions you need to protect yourself.

Can I Use These Agreements for Online Programs?

Yes, the agreements can be used for online programs and provide instructions each step of the way for how to do so.

Still Use This Bundle If I Am Also A Licensed Healthcare Provider?

If you are not acting as a licensed provider while you are coaching these agreements are appropriate for you. However, if you will be acting as a licensed healthcare professional and providing services within the scope of practice of your license these agreements are not appropriate for you.

What is an Authorization to Release Personal or Medical Information and Why Do I Need That?

Sometimes clients want you to share the personal information they share with you with their healthcare providers. Other times, clients request that their healthcare providers share their medical information with you. These forms ensure you are receiving and sharing information in a lawful, transparent way.

What if I Change My Mind Can I Get A Refund?

Due to the immediate, direct access to the digital templates, we do not offer a refund of any kind. Additionally, if you sign up for a payment plan, we do not allow you to cancel the remaining payments for any reason. Why Do I Need All of These Agreements? Can’t I Just Use The Services Agreement?

Each Form and Agreement works together to ensure every piece of your program is protected. Your services agreement ensures you get paid for your program, and that the boundaries and details of your program are clear and understood; the release of liability ensures the client understands all of the risks associated with participating in your program and waives liability; the privacy policy ensures you have a clear policy for using and storing your clients information; and the authorization forms ensure you are collecting or sharing sensitive information in a compliant and clear manner.

How Do I Access the Documents Once I Purchase Them?

A link to the documents will be sent to your email shortly after you purchase them. The link expires after 24 hours. If you would like to access the link after 24 hours click on the original link that was sent to you. It will tell you that you can no longer access that link and shortly after will send a new link to the contracts. There is no limit to the number of links that can be sent to you, but each link expires after 24 hours.
